There should be a ban duration selection, like this:

Temporary
For a limited duration
Permanent

The datetime selectors should only be active when the ban is not permanent.

Currently there is no easy way to tell whether a ban is time limited, you have to compare Start and End datetimes.

As an alternative there could be a "Expiration" check box. Two additional check boxes for Hash and Address would also make sense.
